You have a good point.
An article in the NY Times just yesterday observed that being of "normal weight" increases the risk of a heart attack; worse is being less than normal weight. Being just overweight is the place to be.
You pushed it too far when you wrote:
I'd say todays problem is not so much obesity but what we determine as obese.
because obese is not just overweight, it's dangerously overweight, giving the word its intended meaning.

But you're wrong about diet and exercise being effective.

Another article in the NY Times spoke about the success of diet and exercise in reducing weight and keeping it off. Yo-yo gains/losses are themselves dangerous, so "keeping it off" is an important qualifier.

A Doctor whose career is working with the obese gives the failure rate of diet and exercise as 98%. That's in another NY Times article, this one about the effectiveness of paid diet programs.

Why should they fail, when obviously burning more calories than one takes in is eventually going to reduce weight?
Because as soon as the individual stops pushing himself strenuously and enthusiastically, the effort stops and the weight piles back on.

Why should someone resume an activity with unsatisfying results?
That's the psychological issue that varies with individuals. There may be a lot of commonalities, but it's going to be solved one person at a time. No control, just encouraging people to solve the underlying problem.
Disappointing to the petty dictator, healthy to the individual.
