Too many attachments
Part of the reason for the proliferation of documents is that every time you attach a document to an e-mail message you are making a copy. As copies are edited they become inconsistent.

If you send a document for review try sending a link rather than an attachment. Recipients access the same document rather than inconsistent copies. If the document is updated then the e-mail still refers to the latest copy rather than to an obsolete version. This also allows file system interlocks to work, warning a recipient if somebody else is editing the same <ocument.

It would also help if the filing system allowed you to save multiple versions of a file without having to choose a new name for each version. For instance, VMS allows files to share the same name, differentiating between them by version number. If you try to access a file omitting the version number you access the most recent version. It allows relative version numbers - e.g. version '-1' refers to the previous version of a file. Users can also choose how many versions appear in a directory listing which helps reduce clutter without having to move or delete old versions.
