Both now
Because of multiple sclerosis (the other MS), I can make no productive use of the keyboard. But I can write legibly using an oversized stylus. My Tablet PC not only renders my handwriting with nearly perfect results, it understands stuff even I can't read immediately after writing it.

On my desktop system, I rely upon a non-Microsoft product, Dragon NaturallySpeaking, for all aspects of controlling the system. It did require nearly an hour of training, but Dragon is fast, and gives me better then 95% accuracy. For most purposes, I'm nearly as fast using speech as I was when I could type... for straight text entry, I'm much faster.

Speech recognition and handwriting recognition as genuine productivity tools are here today. I easily posted this particular message using the latter while sitting in my living room.
