It's not an IT problem, it's a business problem and IT projects are no different from non-IT projects. I have been designing and developing very complex systems for 40+ years (IBM mainframes, BTW). My projects have ALWAYS come in on time or early, at or under budget and been extremely reliable - some have run well over 20 years before being replaced with less reliable systems because some clueless VP decides to go with the latest buzzwords. The problem is there are too many projects for the limited number of competent Systems Designers, so 90% of the projects get done (badly) by idiots.
In 1967, an IBM VP told me that by 1970 the country would need 50000 programmers. I replied that there weren't 50000 people in the country capable of being GOOD programmers. We were both right - we got 5000 good programmers and 45000 bumblers.
The more things change, the more they stay the same...
