Filtering works 100% if you want that
If you want to restrict an account to just Disney.com you can do it, so the choice is there, you could restrict your childs Internet to a trusted set (which of course also stops IP proxys and everything else) and thats 100% effective. If you want to supervise their surfing you can disable to filter or turn it down to 'web minus known adult sites'.

The only change I would make is to make it compulsary across all ISPs and Server side.

'All ISP's' so that every parent has access to a server side solution not just AOL.
'Server side' because its easy to setup and impossible to bypass by tinkering with the PC.

The anti-porn lot (includes the DOJ) have hijacked the issue to be about porn, but there are lots of discussions about wars, medical conditions, animal slaughter practices and so on that some parents want blocked, and so they should be able to edit an exceptions list on their filtering option.
