Sometimes God is a 'disgruntled employee'.

Some jobs require a level of authority which also confers the power to destroy. There is no solution which is not vulnerable to being bypassed. It's not really an IT problem, per se.

The communication interface is vulnerable, via packet-sniffing, keystroke loggers, passwords left written down carelessly or simply peering over someone's shoulder. There are steps you can take, but limits to how much you can do. Common sense is the best help here.

Aside from the aforementioned, when was the last time
you heard of anyone hacking into a mainframe OS/390 or z/OS?
