last time I checked, most spam was from the US
I put Spamassassin on exim, but by far and away the best thing for killing spam, is just to check spamhaus.org and reject anything blacklisted. This removed just about all of the spam.

(Spamassassin is no longer so effective since spammers tune their mails to look as little like spam as possible).
