You have to think of it in terms of concentric rings of infrastructure availability, both in terms of actual, physical availability and monetary availability.
On the very outside, you have people who don't even have telephone lines (because of cost and/or lack of physical plant), let alone cable or DSL lines. On the next ring in, you have people who have telephone lines, but don't yet (and may NEVER have) high bandwidth availability (again, cost and/or lines). If you have the phone company eliminate their regular telephone lines, then you have eliminated this second ring of people from having telephone AND Internet access.
