I have worked on a number of large projects (usually government) where the "customer" took so long to make up their minds, changed their minds, or enforced whatever hairbrained ideas they had onto the project which resulted in delays, near failures and overruns which had absolutely nothing to do with the competance of the tech's tasked with implementing the final solution, or meant it was implemented with incorrect or obsolete technology.
