Cell long distance is free too!

Maybe your service is different than mine but there is no charge for long distance on my cell network except for International calls. The premium that I pay is for wireless connection and convenience. If you choose to pay more for long distance it's not the fault of those who choose not to because they have other options that they can use!

And what about accessing the Internet on my cell phone? I pay a premium to the cell company for the bandwidth that I use whether it's a per kilibyte fee or a fixed rate that allows X megabytes per month. But they don't charge the services that I access for them to allow me to get their content.

Consumers used to be miffed that they had to order phone service in order to get DSL. Some carriers now provide "naked" DSL with no phone service although you may pay a few dollars a month more to get it packaged that way. It's still cheaper than paying for wireline phone service, much less long distance charges on top of that. This was brought about by competition, not by regulations.
