So who didn't see this coming? It's simply foolish to rely on 3rd parties for mission critical data or processes. You'd have to believe salesforce has a fair amount of redundancy and fail-over built into its datacenters and service points, yet this still happens?

One of the forgotten points is that business know their own users and business processes - even when such can be somewhat genericized and externalized to vendors like salesforce, the actual business is best at knowing things like when peak usage will likely occur, any special events or promotions that will affect usage. It's not possible for the host to know all the intricacies of your business, and that's one very good reason for keeping critical items in-house - you can plan for business events and be ready...and also not be impacted by another business' events that a host was not ready for.
