C-Band's been doing this for years
C-Band (you know, big dish out in rural areas) has been a la carte for decades. You call up your favorite provider or providers, tell them what channels you want, give them the ID of your receiver, and give them a credit card number. Most "cable" channels are $1-4/month, premium channel bundles are about $10/month. Oh yeah, they have packages of channels, too. Usually you need to get at least 5-10 channels or a package for a provider to do business with you.

I get my analog channels from one provider and my digital channels from a different provider. I pre-pay for a year of programming at a time from each to get a discount. My total bill comes out to something like $30-35/month for all the TV out there that I care to watch.

Now, I'm not suggesting that everyone go out and get C-band, but this just goes to show that the pricing model is already out there. If you're interested in C-band, though, google for "4dtv" to get started.
