Storage is the key
There are any number of potential deliver mechanisms. Broadcast tecnology (either over the air or the internet) can efficiently deliver the same content to millions of "subscribers" at a time. The problem has been the clunkiness and other limitations of tape-based timeshifting.

Disc-based solutions makes it possible to use trickle-down feeds. As Voska says, all viewers requires is that the program be available when they want to watch. The show can come in at 3 am and be locked until 8 pm (or whatever) by the digital rights management system. In a few years, flash memory will also be an economically viable option, adding portability to the equation.

With enough space it becomes easy to capture programs which "might" be interesting.

Digital rights management and WiFi will also make it possible to give the consumer the option of viewing a commercial-laden version for free or paying to watch it commercial free. Advertisers will presumably be willing to pay a premium if they can target ads to the viewer's profile.

As they say, "you ain't seen nothin' yet."
