DirecTivo (where the Tivo is inside the Sat Receiver) is a direct-pay to DirecTV and only costs $4.99 a month. There are no other fee's to pay. It is, in fact, the cheapest way to get Tivo.
Plus, DirecTivo has dual-Tuners. You can record two shows and watch another already recorded show all at the same time.
Finally, the best feature of DirecTivo is the recording is done by recording the true satellite feed signal. Unlike external Tivo boxes, the signal is not decompressed, re-compressed, and then de-compressed again. DirecTivo has a far superior picture over external Tivo's for this reason.
