You can have burned out pixel on CRT. For that to happen you'd have to actually break the tube. Small non visible pin hole would do it if that's possible. You need something to disrupt the phosphorus. The electron guys would go before the phosphorus. I've seen CRTs in use for 30 years. Often you an odd coloration as the one of the red, green or blue electron guns goes causing a usually tinge of purple or yellow and I have seen green once.
