I always keep mine low
Not just because loud music hurts, but because it really annoys everyone on the train next to you.

I have the 'in-ear' style headphones (the ones you have to cram right in your ear) and I barely have the volume about 30-40% of what my Zen is capable of (no, iPods aren't the only MP3 players around!).

I think people have the volume up so high because the headphones that come with most MP3 players are so naff that you have to have it loud to be able to hear the bass of the music...
