Let me modify my position slightly
Thinking about why other professional services are cheaper than patent office, one thing springs to mind:

Competition!

How about it, different professional bodies can issue patents, they take part liability, decide the terms by which they check the patent and a market is created.

If one set consistently issues bad patents then that counts against them because they lose more lawsuits, they have to charge more to cover the cost of the law suits.

It would work as long as they pay the penalty for overturned patents. This is similar to professional bodies, some bodies have a good reputation and pay less insurance as a result, they achieve a status that has value and is worth protecting. The patent office is just a wild card, no control, no liability, issues patent for swinging sideways FFS, nothing like other professional bodies...
