Think of it as a negotiation.
A US company has patented the If, but a European company has patented Then. They agree to exchange patents without charge, and the world continues. Any big, lasting company has accumulated a large number of patents, and can trade with every other big, lasting company.

Where this gets difficult is when a company has gone out of business and sold their patents to an IP operation with a large staff of attornies. Then, yes, some cash could change hands. You can already see the huge value of patents as they're sold like any other commodity.
But then, big companies can afford a reasonable licensing fee.

That's the way it works in the biggest market in the world, the economy every major company wants to be in. And Europe can and probably does operate the same way.

There'd been some discussion of developing countries ignoring the system, but they, too, have to export. The US has a lot of patents, and they insist on enforceable reciprocal agreements on IP.
The key development in IP is the end of resistance, sometimes one country at a time, sometimes through organizations.

Why on earth would you want European countries to be left out of the future?
