IT creates aggravations for users by
- preventing them from personalizing
- ignoring user input
- limiting pc capabilities to specific jobs.

Voska's right that users create problems for IT by
- ignoring rules such as licensing
- adding spyware and malware
- making errors that break the system.

By the way, speaking of breaking the system, the same employer who controlled screensavers so tightly issued instructions to all users about how to clean up junk files manually. This meant going into the Windows folder, for instance, and deleting the contents of certain subfolders.
Making repairs to systems missing essential pieces became the daily work of many until the fun was lessened by one of the free cleaner programs.

Back on topic, the best solution is coordination between users and IT on a case by case basis. I'll use the word "informal" to describe this structure, because "formal" procedures usually are those evolved by IT to enforce inaction on user requests and suggestions.
The problem is not so much to create a smooth mechanism, as to assure that certain users and IT staff (they're in every organization) do not do what they decide is best.
