Doctors are a good example

The GP Doctor now refers patients to specialists. HMO's often make him the gatekeeper authorizing trips to the specialist. Fits the general contractor model we're discussing in the other thread.
And, after a while in which many Doctors became GP's, they're now going into specialties in droves again.

Which would be cheaper for you, having all the specialties in-house, or bringing in someone as needed for some infrequent but substantial services?
Not everyone has to be a specialist, but having someone available when good general knowledge isn't enough seems worthwhile, in principle.
