It can work with an existing POP or IMAP account, (IMAP better).

On the Tools menu, select "Junk Mail Controls", enable Adaptive filters on the filters tab. Set your Junk mails controls:

I have mine set to 'move Junk to the Junk mail folder' and to move items I manually mark as Junk to the trash.
To automatically delete 1 day old Junk, and Sanitize HTML of images when showing junk messages.

You train it for a few days, marking messages as junk simply by clicking next to the message in the Junk folder and after a few days of that your email will automatically be filtered.

I start it, it connects, there are 144 messages waiting.
Next a lot of blue junk icons appear to flag the spam. I'm left with 30 or so messages, of which about 4 will be junk that wasn't trapped.
A quick look in the Junk folder and there is nothing that looks like legit mail (it errs on the side of safety).

Spam problem solved.

I'm not a big fan of spam filters at server side (well except for black lists of known spam servers).

They usually don't know your address book, so have no valid white list.
They aren't trained by you so don't know what you consider spam and not spam.
A false negative client side and you can look in the Junk folder for any emails you think you are missing, a false negative server side and you lose an important email.
