Really needs a paper trail

Yep, but open-source is not exactly a total fix either. Since anything you can write to a disk, I can change with a program regardless of how clever you are with the checksums, logs etc.

Making all the software open source, doesn't stop me running an extra little program to fix the election result.

I really think a paper trail on security paper is needed. Paper because its human readable, so the person making the vote can see his vote registered correctly.
Security paper, because anything that can be printed can be reprinted, so you want it to be as difficult as counterfeiting.

Nothing really complicated there, simply print the votes out on security paper and let the voter see the vote that was just cast/just printed to check it registered their vote correctly.
