Patents are renewable in that the holder of the patent must pay to maintain the patent. That does not however extend the life of the patent. Patents have a certain lifespan and are not extensible.

As for medicine costs, that is something else again. A former rommate of mine works for a major pharmaceutical company. His job is obtain patents for the company. His biggest gripe was that the patents expire after only 20 years. They do attempt to improve processes and patent those as well. Essentially, they patent a cheaper way to make the same thing. That way when the original patent runs out, they can still make the drug for less than any other competitor. However, the original patent does expire, letting others use the process described by the patent.

As for changing formulas, that is true, but a different formula is just that - different. Therefore, it becomes patentable, but it must be a new patent. Just because it's a new patent, it does not extend any protection to any other previously patented process/formula. I.e., if you use the same patented process to make formulas A & B, and the patent for A has expired, anyone can use the process to make A, but not B.
