Quite simple really...
Testing environments are different from User environments. Any developer, project manager (who manages developers) etc will tell you the same story.

It can work perfectly for a programmer, but when a user clicks on something the programmer never thought about.. Wham the whole program goes into oblivion.

No one can think of everything, which is why they need many eyes to look at it in order to get it as debugged as possible from a user side of things.

(Obviously not security side of things because realistically that would entail removing ActiveX, or atleast by default disable it).
