Yes, cities.
The poster asked why big cities couldn't have this service as does Japan. As I remember, most of the large infrastructure upgrades in foreign countries were paid for by their respective governments either directly or through incentives which generally decreased the amount of funding that was available to other areas.

Yes, the telephone companies will do the heavy lifting, but they won't pay for it unless it will make an obvious and quick return on their investment. Shareholders generally demand nothing less. Otherwise, it will be left up to the cities who will, in one way or another (either through generous tax write-offs or other forms of graft) pay for it.
