it's being done
i work for a relatively small, yet nationwide ISP, and yes, we HAVE been kicking people offline. if our mailserver notices huge amounts of activity from one computer, we'll take a look at it. it's obvious who is infected with some spamming worm or virus, and we simply turn em off. no notice to them whatsoever, if they call up complaining, we tell em "you wont be turned back on until you update your antivirus, do a full virus scan, and come up clean"
