Sometimes a refusal to listen . . .
Sometimes it's a refusal of top level management
to listen to lower level employees as well. NASA
was notorious for having unsatisfied employees who
knew key facts about issues, but ignored them,
even if the employee was proactive about trying to
contact somebody about it.
