HRM Failure
The biggest missing category/type is human resource management failure, to include poorly defined work, roles, KSAOCs (the human characteristics needed to accomplish work effectively); poorly selected participants as to their KSAOCs; poorly designed/executed incentives (could be routine total compensation plans or those specific to the project); poorly designed/executed work environment/culture/employee relations; and the list goes on. Failures of leadership, whether or KSAOCs or execution, always belong on this HRM list. Almost every project or other business failure that I've observed in a (some would say too) long career can be traced to failures of human resource management, whether within the organization itself or within partners/vendors/etc.
