I agree that most of the failures are not
because of technical issues. Project managers
often focus on the mechanics of managing the
activities. There needs to be a focus on
business value. This can only come from the
business.
Adding to the Stakeholder involvement item you
mention, I have seen many project managers
"involve" the stakeholders by letting them
see/hear what's happenning, but there is no
accountability on the stakeholders. It is a
one way contract, what IT will do. It needs to
be a two way contract with the business needing
to deliver something. IT can deliver the
technical components.
