What does "Did not deliver as promised" mean?
The study reported on considers reasons to cancel a project, not to declare one a failure after completion.

If a project is incomplete, it has failed to deliver by definition.

My best guess is, this refers to projects which had failed to meet deadlines. If I'm right, then the discussion of this reason might better concern what slows a project down intolerably.
