Communication is the overriding key factor.
The biggest IT failures are a result of not communicating effectively.

So do it, communicate clearly and fully. I mean communication in all directions: internal and external and both up-stream and down-stream management.

Most importantly, include the people who will be using the system! Include them at *every* step!

Seriously, listen to your intended users!

Listen to their input during the requirements specification phase, ask to ensure that you understand their needs, and ask again even when you're sure you do. Involve them in the design phase, the pilot, the beta-test, the release candidate, and every other possible step that you may take.

Don't discount their importance. The system you're creating is *for* them!

Most project failures I have observed come from systems which don't meet their user's needs or meet them in a complex and/or confusing manner. These systems get bypassed and human intervention processes evolve to work around them... resulting in a massive waste of resources and lower productivity -- exactly the problem that the IT project was intended to solve.

Just my $0.02 USD based on 30 years in the business... you're opinion and experience may differ.
