An Unfair Comparison
The comparison is somewhat unfair. When the wrong patient gets an appendectomy, or the wrong blood, or instruments left inside the patient, it can be clearly established that the mistake stemmed from

* Carelessness
* Some best practice protocol not being followed
* Lack of experience or expertise
* Gross negligence

in other words, malpractice.

In IT projects, it is not unheard of for unrealistic contracts to be accepted by the vendor in order to win the contract. Trying to educate the prospect that the project being envisioned is not feasible often pisses the prospect off ("you (the vendor) are not listening to me").

I would guess that most people, once they knew they had appendicitis, would not insist on having only part of the appendix removed ? especially after being informed as to why that would be a bad idea and why an appendectomy a good idea.

I?m not saying that a consulting vendor never has extended the project for higher billings. I?m just saying plenty of flawed decisions get made by the client when defining the IT project
