Project Manager Usually Ends up "Holding the Bag"
The project manager is often not the one who sets the mission. That is usually done several layers up the management chain. The project manager is often not in a position to question the mission either.

With projects that fail miserably, the mission and project expectations are flawed in some severe way from the get go.

Usually, the project manager is focusing on meeting the project goals (and not the mission goals) because that is what is expected of him/her. Which often leaves the project manager "holding the bag"

Regarding Baseline's comment on planning, well, you need a realistic plan and approach for getting project done. Sh*t happens. Therefore, you don't want to start the project on shaky ground. If you want to get to NYC from Toronto, you don't want a plan that gets you to Pittsburgh,

You also need to be able to tell how the pieces of the project are progressing and their affect on the whole project. You need to figure out what areas (tasks, people, systems) need to be supported before small issues become big issues.
