Your doctor doesn't accept appointments? S/he told you never to call and have a physical? With all your technogadgets, you can't program one to pop up a "call your doctor" reminder? You can't program your significant other to nag you about your health? Face it, it's your fault, not your insurance company's. Although you do seem to be the kind of person who would welcome someone showing up on your doorstep saying "Hi, I'm from the government and I'm here to give you a colonoscopy."
