Today, I suspect, malpractice insurance is the primary reason US physicians do not go hang out a shingle and work independently. If malpractice insurance could be reduced for those willing to diagnose and treat by computer then I think computerized doc-in-a-box will really take off. There are other threats to the profession - other licensed professionals and medical herbs that when legalized draw patients away from MDs, which can not sell drugs by law, to them.

And there are plenty of MDs coming out of the military who prefer being employees rather than entrepreneurs. The profession is being hit on many sides. It is not just automation of the diagnosis process. Then when you compare salaries of similarly educated practitioners in other countries you start thinking of overpricing.
