Not enough thought in designs
There's so much that could be improved with an extra week's toil on careful initial design and studying usage. My graphics professor once pointed out how silly doors are designed throughout the older buildings on the university (push, or pull, or slide, and which way, and how can you tell?). Now I can't help noticing how braindead sometimes we are in making even the simplest things: unlabeled switches and knobs, hidden buttons, controls that aren't placed next to the things they control, invisible features, and so on and so forth. How many times have we turned on the wrong stove on the oven or pushed/pulled the wrong way on a door?

And of course, just saying it doesn't create reality. Supposedly, Microsoft forces everyone to read the book "The Design of Everyday Things", but it doesn't seem to have helped that much....
