GotVMail
I've used GotVMail (they are, in their words, a "Virtual Small Business Phone System" provider) for over a year - and they are both my 'local' phone number provider and a toll free number too. I eventually 'replaced' my only phone numbers (a home phone and cell phone #) with my GotVMail-provided local phone number and I have not had a single problem, reliability-wise, billing-wise, etc. Of all the players (without knowing Skype), I think GotVMail is by far the longest-established (they've been around since 2003) and have the 'cleanest' Better Business Bureau record. So if I had to recommend using a reliable provider to replace your POTS line, etc., it would be GotVMail.
