Asterisk
Asterisk, of course, with a little bit of intestinal fortitude can be configured for the home and I might add, I have it configured for my IT consultancy to have my Nokia N95 SIP-cable cell phone answer as a remote extension from 'anywhere'. No cellphone plan minutes are used and the SIP calls are over VoIP on a flat-rate monthly data bearer plan.

Of course, Apple could put together the code, but they have a standing reputation for taking what is good and forking it.
