NASA study
Remember that the main issue NASA had with releasing that study was that its results are meaningless, as the study failed to eliminate double- or triple- (or more) reporting of incidents. What's most embarrassing is that so much money was spent on a study with an inherently flawed methodology. Oops. Any resulting panic or concern about the number of incidents would be entirely inappropriate, since those numbers are just wrong.
