Cost is still too high / some technical problems yet
Based on my experience with companies as GENCO (one of the worlds largest logistics and reverse logistics companies) the cost of the passive chips are still over 50 cents each. This makes it prohibitive for the mass volumes of items and is reserved for only the most expensive or grouped item tracking. Until it gets to about 5 - 10 cents each, it will never get widespread implementation - regardless of what organizations as Walmart try and push on their vendors.

As a note, GENCO currently has over 1,000,000 bar code scans per month in tracking returned items (3PL operation) and would love to replace. They are in the process of testing several varieties of equipment and both active and passive RFID systems but believe as I do about wide spread adoption.
