Factors on small business acceptance
For many small businesses, they've already made the investment into a phone system, and the rates they get a pretty cheap. A 10K phone system investment is nothing to sneeze at for a small company, so throwing it out to go VOIP a hard sell. The other issue is availability of service. Adding the VOIP system in adds another layer of complexity. Running a extra phone line to your house or maybe supplimenting your cell phones for at home is one thing, but when your business depends on the phone system, having a 99.9999% availability on your bandwidth provider as well as your VOIP company is crucial. Until you can nail down a provider with an ironclad SLA for both bandwidth and call termination, it's really a fight.

Just because you can pretend to be a phone company is one thing, actually getting service and availability to match is another.
