Actually, it's rather easy
A home telepresence system needs four things: A big, bidirectional pipe, a transceiver, an HD camera, and an HD display. Many homes now have an HD display, and a cable internet connection has a high-speed pipe one-way. HD cameras are coming down in price, and set-top boxes are becoming more integrated and feature rich.

So what is needed is for cable internet access to become less asynchronous and provide higher upload speeds (at least 1Mb/sec), then for Cisco's Scientific Atlanta division to create a cable set-top box which has the necessary electronics and network quality of service features to support telepresence, and finally an HD "webcam" to sit on top of the HD display.

The result would be the ability to join living rooms, so grandma can see the grandkids, etc.

This would be a killer app for all of the HDTVs out there, most of which are just watching SDTV stretched to fit the wider screen.
