More than PPV
I rarely do PPV or BlockBuster (I'd still rather order a movie from my living room than go out in -15 windchill weather. And for the same price.). I do use Video On Demand (VOD) though. It doesn't cost anything extra. I can select a show, pause it, fast forawd, rewind, etc.
A number of channels have this: Answers, Anime, A&E, BBC America, Cartoon, Comedy Central, CNN, Court TV, DIY, Food, Golf, HGTV, National Geographic, Oxygen, PBS Kids, Speed Channel, our local cable news, Movie Channel, Music, Info Channel, and 2 local In Demand HD channels with not-newly released movies.
HBO and Showtime are available On-Demand, but you have to subscribe to the channel.

Also, how do you record to a PVR (or TiVo) without the interactive menu?

I also didn't buy a settop box for $200. I pay a couple bucks to the Cable company per month.

I understand different people's needs and cost issues. Some people are happy with dial-up, and can't justify broadband. But everyone I know that has tried it didn't go back.
