A (Possible) Solution
This was alluded to earlier. Why not have a public/private telephony network consortium made up of fiber optic lines, switching systems, transmission systems and such. This network would "wire" all homes and businesses enabling the consumer to pick and choose their video (TV), audio (radio, etc.), telephone, and internet access from any and all providers located anywhere in the country.

In essence, this is what is begin done with deregulated electricity and natural gas. The consumer can choose which power or gas provider to buy each from and then the provider leases the transmission line company to send it to the consumer.

The old monopoly was that the whole package was owned by one company like most cable companies today. So long as they were making money, the company had no incentive to change, improve and lower prices.

With the network owned separately from the various service providers, the providers would need to compete with all the other providers and thus be forced to keep themselves lean and mean or loose out.
