Divest the infrastructure
It's long past time the FCC and other government agencies should have forced the cable and phone companies to divest themselves of the physical infrastructure (the wires and cables). An independent company should own these assets and be required to open them up to anyone desiring to deliver services - whether those services are POTS, data, television or whatever. This is the model for electrical grids but unlike utilities there are lots of providers and delivery of services can be made to individual customers rather than whole cities.
