This is actually part of the problem
I don't know what the best answer is and I've long thought about how best to address it. If you have one entity that controls both the distribution medium (cables) as well as access (the service) that creates the monopolistic problems we have.

Maybe what is needed is for a third party (but not the government) to take control of the distribution means (cable and telco) but not sell services and have companies then purchase bandwidth to resell from that entity so that it re-invests into the infrastructure. I'm sure that this has many holes in it. But something sure needs to be done. In my area, I can get (from only one provider each), cable modem service (6Mb/256-lots of ports blocked), 8Mb/768 DSL (wide open so far-and we also can de-couple the POTS so we have "naked" DSL, but that's only recently), and limited broadband WiFi-which is not really a third choice since it is only offered by -yes one of the telco providers (although I can choose AT&T, Verizon, or Sprint so at least there is some choice there-but coverage sucks unless I'm in the metro area).

But I have to say that part of the problem in the US is because of regulation not lack of it. That's why I think we need to remove regulations, create a non-government third party infrastructure ownership and then sell access.

OK, so hit me with the flaws in my comments-and maybe an idea as to what can be done (assuming we ever would have the power to do something that the monopolies would not block by congressmen/women in their pockets).
