If WiMAX becomes ubiquitous, as long as there is someone within YOUR visual range - you will be connected.

Quantities count. If that poor SOB is part of the chain between San Jose and Monterey, his dang router is going to slag under the load without him being able to get a packet in edgewise. Meanwhile, the people in Monterey are going to be better off with dialup.

Hawaii is still going to be a problem.

Since counties are about the same size as WiMAX range - you would need at a minimum ONE to cover the county.

Only in areas with extremely high population density, in which case you don't need them anyway. Your idea of a county is rather small -- mine has an area of 9226 square miles.

In mountainous areas, you place them on strategic high points - where your other broadcast antennas are ALREADY located.

Again, you seem to have a rather romanticized notion of mountainous areas. Much of the western USA (and a large part of the Appalacian stretch) doesn't have line-of-sight to anything farther than a couple of kilometers at best.
