[I didn't know it went BVR (Beyond Visual Range.) How does it get over the horizon?]

If WiMAX becomes ubiquitous, as long as there is someone within YOUR visual range - you will be connected. This works for 90% of the population that is close to cites. As for the rural 10% you will have the same issues as EVERY OTHER service vendor has - which is low population density makes coverage tough. These rural governments could place WiMAX repeaters at regular intervals (using MUCH less than WiFi). Since counties are about the same size as WiMAX range - you would need at a minimum ONE to cover the county. In mountainous areas, you place them on strategic high points - where your other broadcast antennas are ALREADY located.
