The telecoms own the backbone, pulled the fiber (long haul), supply the addressing (phone numbers) and handle QoS for the long haul. Your company goes through some service for network access, an example of this is the (pre standard) WiMAX service just started in Seattle, Wa. This is based upon the Space Needle and the service costs $800/month (to businesses).
You get telecom via either a wire line or a RF signal, if you don't then you can't do address resolution (dial a number, connect, QoS, etc.).
