"reasonable precautions" has nothing to do with it
It isn't about "reasonable precautions"; it is about "all precautions".

The details vary depending on whether this is IIS or a Unix/Linux system but the fact is that these systems can be locked down and kept locked down. It requires competent and alert management, with continual system monitoring to determine whether the system is becoming compromised.

But if the administrator staff knows its job, and is paying attention, a system is unlikely to be compromised. If it becomes compromised anyway, that fact will be evident very quickly (minutes, probably) and the problem can be corrected - with the very first step being to physically unplug the server from the network.

These types of thefts always occur because the IT staff was either lazy, or overworked, or just not competent.
